PA Home Care Through Medicaid Waiver - How To Apply



  1. Determine if you are eligible for Medicaid in Pennsylvania. You must meet certain income and asset requirements. You can check your eligibility and apply online through the Pennsylvania Department of Human Services website (https://www.compass.state.pa.us/compass.web/Public/CMPHome).

  2. Once you are enrolled in Medicaid, you will need to apply for the Medicaid waiver program. There are several different waiver programs, but the most common one for home care is the Aging Waiver program. You can apply for the Aging Waiver program online through the Pennsylvania Department of Aging website (https://www.aging.pa.gov/Pages/default.aspx).

  3. After you apply for the waiver program, you will need to undergo an assessment to determine your level of need for home care services. This assessment will be conducted by a representative from your local Area Agency on Aging (AAA).   4. Once you have been assessed and approved for home care services, you will be assigned a service coordinator. This person will work with you to develop a plan of care based on your individual needs and preferences.

  5. Your service coordinator will help you find a home care provider who participates in the Medicaid waiver program. You will need to choose a provider who meets your needs and preferences, and who is willing to accept Medicaid payments.

  6. Once you have chosen a provider, you will need to sign a service agreement with them. This agreement will outline the services you will receive, how often you will receive them, and how they will be paid for.

  7. Finally, you will begin receiving home care services from your chosen provider. Your service coordinator will continue to work with you to ensure that your needs are being met, and to make any necessary adjustments to your plan of care.
