Independent Home Health. ensure participants understand that their information will be available only to following parties outside of their written consent: •The participant •The participant’s legal guardian •The provider staff for the purpose of providing a service to the participant •An agent of the Department •An individual holding the participant’s Power of Attorney for health care or health care proxy •A court ordered request Independent Home Health will provide all participants and parties with whom the organization has a direct relationship with a copy of the organization’s Notice of Privacy Practices.
